• Support the Integrated Care Team by reducing social barriers to health.
• Coordinate connection to community treatment resources.
• Coordinate hands-on clinic care with patients, PCPs, BHCs, and other providers.
• Maintain patient registry and report to care teams, clinical team leads, and Social Work Lead.
• Serve as liaison with mental health and social service partners in the community.
