Oak Street Health in Memphis, TN is seeking a Medical Social Worker to support the psychosocial needs of our patients. You will participate in assessments, advocacy and comprehensive care coordination to help patients reduce hospitalizations and improve health outcomes.

In collaboration with the care team, you will educate patients about care options, assist with insurance navigation, and connect them with community resources. Strong communication in multiple languages is valued.

Qualifications

MSW degree from CSWE-accredited program.
Current licensure in the state of OSH center.
Fluency in Spanish, Polish, Russian or other local languages.
Proficient computer skills and EMR experience.
US work authorization.

Responsabilités

Provide assessments, advocacy and care coordination for high-risk patients and caregivers.
Collaborate with interdisciplinary team to support psychosocial needs.
Educate patients on care options, advance care planning and goals of care.
Navigate insurance benefits and maximize plan utilization.
Document interventions concisely for all care team visibility.
Engage with Community Health Workers for complex patient needs.
Respond to crises with prioritization and multitasking.
Other duties as assigned.
