Oak Street Health is seeking a Medical Social Worker in Memphis to support psychosocial needs, conduct assessments, advocate for patients, and coordinate comprehensive care with the interdisciplinary team. The role emphasizes collaboration with care teams and community resources to prevent hospitalizations and improve health outcomes.

Responsibilities include comprehensive assessments, care coordination, education on support options, and crisis response, reporting to the Practice Manager.

Qualifications

• MSW degree from CSWE-accredited program.
• Licensed in the state where OSH Center is located.
• Fluency in Spanish, Polish, Russian, or other community languages.
• Proficient with Microsoft Office, G Suite, and EMR systems.
• US work authorization.

Responsibilities

• Proactively support highest-need patients with assessments, telephonic check-ins, and in-person visits.
• Collaborate with the patient and support persons to identify goals and drive care plan execution.
• Maintain knowledge of community resources and refer to meet healthcare and psychosocial needs.
• Navigate insurance benefits with the Patient Relations Manager to maximize benefits.
• Provide longitudinal and episodic care coordination to empower patients to manage health conditions.
