Job summary

Christ Community Health Services seeks a compassionate Licensed Clinical Social Worker (LCSW) to join our integrated Behavioral Health team in Memphis. The role combines clinic-based care at the Third Street Health Center three days a week with mobile outreach two days weekly, serving patients of all ages in diverse settings.

Qualifications

• Master’s degree in social work from an accredited institution.
• Active Tennessee Licensed Clinical Social Worker license or eligibility to obtain Tennessee licensure.
• At least two years of post-graduate experience, preferably in hospital, primary care, or integrated healthcare.
• Experience collaborating with physicians and other health professionals for team-based care.
• Experience delivering evidence-based interventions for mental health and medical conditions with psychosocial components.
• Experience using electronic medical records for clinical documentation and interdisciplinary communication.

Responsibilities

• Provide behavioral health services at the Third Street Health Center three days per week and via mobile outreach two days per week.
• Use screening tools and clinical interviewing to complete assessments.
• Diagnose behavioral health conditions and develop treatment plans.
• Manage crisis situations, including suicidal/homicidal ideation, and coordinate interventions and referrals.
• Deliver evidence-based interventions for mental health and chronic conditions.
• Facilitate cognitive-behavioral and psychoeducational groups.
• Refer to psychiatry and community resources as appropriate.
• Document timely and accurately in the EMR.
• Collaborate with interdisciplinary teams to support whole-person care.
• Provide clinical supervision to practicum students or interns.
