Job summary

Northwell Health is seeking a registered nurse to perform an age-specific plan of care for designated patient groups using the nursing process. The RN collaborates with physicians and health team members, and uses clinical judgment to coordinate care on the unit.

You will assess physiologic and psychosocial health, develop and implement the plan of care, educate patients and families, document care, and participate in discharge planning.

Qualifications

• Graduate from an accredited School of Nursing.
• Associate’s Degree in Nursing required.
• Bachelor’s Degree in Nursing preferred.
• Current RN license in New York State and/or Connecticut.
• BLS certification required; additional certifications as needed.
• 1-2 years of professional nursing experience in the related specialty.

Responsibilities

• Assesses physiologic health status and organ systems.
• Assesses psychosocial health status and needs.
• Identifies patient outcomes and goals.
• Formulates and coordinates the plan of care.
• Implements plan of care and ensures safety and timely procedures.
• Evaluates care effectiveness and documents outcomes.
• Collaborates with physicians and care team; delegates tasks as appropriate.
• Performs preventive skin care and decubitus care as needed.
• Administers medications correctly and safely.
• Performs patient and family education and discharge planning.
• Performs related duties as required.
