Job summary

Elara Caring is seeking a Registered Nurse Behavioral Health to provide patient-centered care across East Cuyahoga, Lake Geauga, and Ashtabula counties. You’ll assess patients, develop and implement plans, and coordinate with interdisciplinary teams in a home health setting.

Must hold an active RN license and be willing to travel up to 50% within the service area. Prior home health or hospice experience is preferred, with opportunities for tuition reimbursement and a strong benefits package.

Qualifications

• Current, unrestricted state RN license.
• 1+ years of clinical care as a nurse.
• Able and willing to travel within branch/office coverage area.
• Must have a dependable vehicle, valid driver’s license, and current auto insurance.
• Ability to frequently lift, push, pull, and support up to 50 pounds.
• Willing to travel 50%.
• Previous Home Health or Hospice experience is preferred.
• Experience with Homecare HomeBase is preferred.

Responsibilities

• Assess assigned caseload of behavioral health patients and families to identify needs.
• Assume primary responsibility for caseload including planning and evaluating the plan of care.
• Assess patient needs and collect data affecting health status to incorporate into the care plan.
• Make initial evaluations and re-evaluations of nursing needs during visits.
• Conduct follow-up visits as required by patient, family, and facility.
• Communicate significant findings to supervisors and team.
• Revise the plan of care as needed and coordinate with staff.
