Job summary

Elara Caring is hiring a Psychiatric Registered Nurse Home Health to serve Amherst and surrounding areas. The role includes conducting comprehensive patient assessments, managing care plans, and delivering behavioral health guidance during home visits. A $5,000 sign-on bonus is offered.

Required are an active RN license, nursing degree, and 1+ year of clinical nursing experience. A valid driver's license or reliable transportation is essential to travel between patient homes.

Qualifications

• Current State license as a Registered Nurse.
• Graduate from an accredited nursing program (Certificate, Diploma, Associate or Bachelor).
• 1+ year of nursing experience in a clinical care setting.
• Home Health experience preferred.
• Must have a valid driver’s license or reliable transportation to travel between patient homes.

Responsibilities

• Assess patients’ physical, emotional, social, and environmental needs.
• Manage caseload and develop, coordinate, and update care plans.
• Complete initial assessments and provide ongoing evaluations during visits.
• Monitor patient progress and communicate with physicians and the care team.
• Update care plans based on changing needs.
• Educate patients, families, and caregivers about the plan of care.
