Job summary

Elara Caring is hiring a Registered Nurse for Behavioral Health in the Worcester/Shrewsbury area. The role offers a guaranteed salary plus a productivity bonus, with a sign-on bonus of $5,000 and full-time daytime hours.

You will support home-based care across the region, ensuring high-quality behavioral health services in patients’ homes. Requirements include an accredited nursing degree, current RN license, and ability to travel within the branch area.

Qualifications

Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.

Responsibilities

Assess assigned behavioral health caseload and identify patient needs from clinical records and evaluations.
Manage caseload: assess, plan, coordinate, implement, and evaluate the plan of care.
Evaluate patients’ physical, psychological, social, and spiritual factors impacting health.
Perform initial evaluation and re-evaluate nursing needs at each visit.
Conduct follow-up visits as required by patient and facility needs.
Communicate significant findings to supervisors, physicians, and care team.
Revise care plans and notify team of changes.
Counsel and involve patient and family in care plans.
Teach behavioral health concepts and pain management to staff.
Supervise delivery of care by aides and LPNs per policy.
Flag unusual facility issues to supervisors.
