Job summary

Elara Caring is seeking a Registered Nurse with Behavioral Health expertise for our Springfield, MA home health team. You will assess patients, develop care plans, and collaborate with physicians to deliver high-quality, compassionate care in patients' homes.

Ideal candidates have a valid RN license, 1+ year clinical experience, and (preferred) home health exposure. The role involves lifting up to 50 pounds and driving to patient locations. Competitive salary and benefits provided.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, or have reliable transportation to travel between patient homes and work locations.

Responsibilities

• Assess patients' physical, emotional, social, and environmental needs.
• Manage a caseload and develop, coordinate, and update individualized care plans.
• Complete initial assessments and provide ongoing evaluations during patient visits.
• Monitor patient progress and communicate changes with physicians, supervisors, and the care team.
• Update care plans based on patients' changing needs.
• Educate patients, families, and caregivers about the plan of care and behavioral health support.
• Teach behavioral health care and symptom management to staff in partner care settings.
• Supervise and support Behavioral Health Aides and LPNs while ensuring quality care.
• Report safety concerns or other issues to supervisors as needed.
• Provide compassionate end-of-life support and complete bereavement assessments.
