CVS Health seeks a Behavioral Health Residential Coordinator to coordinate admissions for out-of-home behavioral health services, serving as a key liaison among utilization management teams, residential providers, and care management staff. It involves managing referrals, tracking progress, and addressing disruptions to ensure timely access to quality care.

Qualifications

• 2+ years of behavioral health experience in an integrated care setting.
• Proficiency with Microsoft Office (Excel) and electronic records systems.
• Willingness to travel up to 10% within the region.

Responsibilities

• Coordinate residential treatment facility referrals and admissions for members in out-of-home behavioral health residential care.
• Identify clinically appropriate participating residential providers to support member goals.
• Manage referral process, distribute referrals, track responses, and document activities.
• Collaborate with behavioral health homes, providers, and partners to submit clinical information for facility reviews.
• Monitor referral status and facility timelines to promote timely access to care.
• Serve as primary contact for providers and internal stakeholders regarding facility coordination.
• Respond urgently to disruptions, denials, or changes by coordinating alternatives.
• Participate in utilization management rounds and case reviews with the team.
• Facilitate meetings with residential providers to improve collaboration and outcomes.
• Partner with Care Management, Utilization Management, Quality Management to support continuity of care.
• Identify and escalate concerns to leadership and maintain compliant documentation.
• Support departmental projects, reporting, and process improvements.
• Apply care coordination principles and established policies to promote quality outcomes.
