Elara Caring is seeking a Registered Nurse Behavioral Health to join our team delivering compassionate care in patients' homes. You’ll assess patients, manage caseloads, develop and update care plans, and provide ongoing evaluations during visits as part of our mission to deliver Right Care, at the Right Time, in the Right Place.

Requirements include an active RN license, graduation from an approved nursing program, 1+ year clinical experience, and the ability to lift up to 50 pounds.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.

Responsabilités

• Assess patients' physical, emotional, social, and environmental needs.

• Complete initial assessments and provide ongoing evaluations during patient visits.
• Monitor patient progress and communicate changes with physicians, supervisors, and the care team.
• Update care plans based on patients' changing needs.
• Educate patients, families, and caregivers about the plan of care and behavioral health support.
• Teach behavioral health care and symptom management to staff in partner care settings.
• Supervise and support Behavioral Health Aides and LPNs while ensuring quality care.
• Report safety concerns or other issues to supervisors as needed.
• Provide compassionate end-of-life support and complete bereavement assessments.

Job Description

As a Registered Nurse Behavioral Health, you’ll contribute to our success in the following ways:

• Assess patients' physical, emotional, social, and environmental needs.

What is Required?

• Current State license as a Registered Nurse.

• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

• The base salary for this position is $85,000 to $90,000 annually based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity. * Benefit eligibility can vary by position and shift status.
