Job summary

Elara Caring is seeking a Home Health Registered Nurse in IL to assess and plan patient care, provide nursing services per physician orders, and ensure HIPAA privacy and safety in a home care setting.

We offer a $5,000 sign-on bonus, comprehensive benefits, tuition reimbursement, PTO, and a 75k–80k base salary with a supportive team focused on quality, compassionate care at the right time and place.

Qualifications

• Active RN license in the state.
• Graduate of an accredited nursing program.
• 1+ year of nursing experience in a clinical setting.
• Home health experience preferred but not required.
• Ability to lift up to 50 pounds and travel between patient homes.
• Valid driver’s license or reliable transportation.

Responsibilities

Job Description

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.
• Protecting patient and staff privacy in accordance with HIPAA.
• Providing nursing care according to the plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.
• Educating patients and families on their care and safety.
• Taking appropriate action when patient conditions change.
• Helping ensure quality, safe, and continuous patient care.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

The base salary for this position is $75,000 to $80,000 annually, based on the company’s good faith estimate at the time of posting.
