You will assist clinicians, manage referrals, and coordinate care while maintaining accurate records and ensuring timely access to services. The role requires strong organization, communication, and cultural sensitivity, with attention to HIPAA and safety policies.

Qualifikationen

• Requires high school diploma or GED; Associate degree preferred or 5+ years in front office medical/behavioral health experience as substitute.
• Experience with case management and care coordination is preferred.
• Bilingual ability in English and another language is a plus, not required.

• Link patients to behavioral health services and coordinate referrals across CHC network.
• Assist with scheduling, documentation, and maintenance of patient records per policies.
• Provide quality customer service and support to clinicians and patients.
• Maintain confidentiality and adhere to HIPAA and safety policies.
• Support clinics with outreach and program coordination as needed.
