Job summary

Luminis Health, Inc. is seeking a Behavioral Health Navigator to connect patients with comprehensive mental health and substance use services across ages.

The role collaborates with medical and mental health teams, community agencies, and primary care providers, communicating with patients, families, and teams by phone to ensure timely referrals within 48 hours. Requirements include a Master’s Degree in Counseling or Social Work and at least two years in behavioral health.

Qualifications

• Master’s Degree in Counseling or Social Work required.
• Two years of experience in behavioral health stated as Education requirement.

Responsibilities

• Provides consultation and education to providers in the implementation and coordination of multi-disciplinary care.
• Provides referrals to the patient for outpatient mental health treatment and substance abuse treatment.
• Serves as a liaison between the patient and ancillary services, e.g., treatment centers or programs as well as serving as the direct contact to the patient.
• Refers patients to other resources or services in the community, such as support groups and recovery groups.
• Maintains a customer-oriented approach to meeting patient needs in an expedient manner, involving the family in the plan of care as appropriate.
• Interviews and assesses patient using behavioral health knowledge and skills in making referrals.
• Recognizes process needs and facilitates appropriate change through collaboration with emergency department and information technology personnel.
• Develops and maintains detailed list of community resources for referral.
• Collaborates with Community Behavioral Health Navigator regarding community resources and accessibility of services.
• Tracks and reports monthly metrics for Bay Area Transformation Grant.
