Schedule: Part Time (20 hours, Days)

As a Behavioral Health Navigator (BHN), you will work with an interdisciplinary team of medical professionals to determine the appropriate care of all patients who are present to the Emergency Department in behavioral health crisis. BHNs collaborate with outpatient providers, DMH, DCF, families, schools, etc. to determine the needs of each patient and to ensure that the patients are receiving the best, most effective care available.

Responsibilities & Duties:

• Perform confidential interviews, both in person and via telemedicine, to assess patients’ needs from referrals directed by Emergency Department physicians.
• Collaborate with health professionals to facilitate interdisciplinary information sharing and treatment planning, uncovering barriers to care and developing effective plans to alleviate these barriers for patient safety and betterment.
• Document assessments, interventions, and plans related to the patient’s confidential medical record in accordance with HIPAA, industry, and compliance standards.
• Provide education/consultation as needed to mental health care team members and health care consumers.
• Maintain current up‑to‑date knowledge of regulatory requirements.
• Educate and support clients to engage in their care, improve health literacy, and change health behaviors (e.g., diet, medication adherence, compliance with doctor’s visits).
• Collaborate with physicians and clinical staff to identify high‑risk medical/social management patients.
• Make referrals to community agencies when appropriate.
• Identify potential legal or risk management issues and discuss with supervisor.
• Perform other duties and responsibilities as assigned.

Job Requirements

Education:

• MSW or Master’s in Counseling.
• Doctorate in Psychology or Social Work preferred.

Certifications, Licensures, and Registrations:

• Licensure at the independent level in the state of Massachusetts.
• Licensed Psychologists, Licensed Independent Clinical Social Workers (LICSW), or Licensed Mental Health Counselors (LMHC) required.

Experience:

• 2–3 years of direct diagnostic experience.

Knowledge and Skills:

• Skilled in assessment, diagnosis, and disposition of behavioral health patients.
• Knowledgeable about services available to behavioral health patients.
• Computer skills essential.
• Ability to work in a fast‑paced environment.
• Ability to communicate effectively with internal and external customers across cultural and value differences.

Compensation Range:

$61,500.00 – $89,500.00

This range offers an estimate based on the minimum job qualifications. Compensation includes a broad range of factors when making an offer, such as education, experience, skills, certifications/licensure, organizational needs, equity, and market competitiveness.
