75% clinical and 25% administrative duties, with leadership in staff development, community engagement, and program integration to support the department’s mission.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, or related field required.
• Minimum 3 years of behavioral health or social service case management experience.
• Strong knowledge of behavioral health documentation and integrated care.
• Excellent organizational, interpersonal, and communication skills.
• Experience linking patients with community resources and patient enabling services.
• Master’s degree preferred; grant management and school-based behavioral health experience a plus.

Responsibilities

• Supervise and support the behavioral health case management team.
• Oversee workflows, ensure quality care coordination and documentation.
• Lead staff development, program integration, and community engagement.
• Participate in Quality Improvement activities and risk management.
• Coordinate care with clinicians, schools, and community partners.
