Qualifications

• Requires High School Diploma or GED.
• Graduate of an accredited Nursing program (Certificate/Diploma/AS).
• Current State License as a Licensed Practical Nurse (LPN).
• 1 year of experience in a clinical care setting; hospice/home health preferred.

Responsibilities

• Verify Plan of Care before each visit and provide care per RN delegation and physician orders.
• Document nursing notes and update medication profiles as required.
• Communicate findings to RN Case Manager, physician, and team.
