Elara Caring is seeking a Registered Nurse Behavioral Health to provide in-home visits in the Bridgeport, CT area. The PRN role focuses on comprehensive behavioral health assessments, care planning, and ongoing nursing support to keep clients safely at home.

You will manage caseloads, coordinate with physicians and a care team, and educate families on the care plan. Prior home health experience is a plus; lifting and driving requirements apply.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.

Responsabilités

• Assess patients' physical, emotional, social, and environmental needs.
• Manage a caseload and develop, coordinate, and update individualized care plans.
• Complete initial assessments and provide ongoing evaluations during patient visits.
• Monitor patient progress and communicate changes with physicians, supervisors, and the care team.
• Update care plans based on patients' changing needs.
• Educate patients, families, and caregivers about the plan of care and behavioral health support.
• Teach behavioral health care and symptom management to staff in partner care settings.
• Supervise and support Behavioral Health Aides and LPNs while ensuring quality care.
• Report safety concerns or other issues to supervisors as needed.
• Provide compassionate end-of-life support and complete bereavement assessments.
