Job summary

Elara Caring is seeking a Registered Nurse Behavioral Health for visits in Hartford, New Britain, and surrounding areas. This PRN role offers flexible scheduling to help keep clients in their homes while delivering high-quality behavioral health care.

You will assess needs, manage care plans, educate families, and collaborate with physicians and supervisors to ensure patient progress and safety. A valid RN license and 1+ year clinical experience are required.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Nursing program (Certificate, Diploma, Associate, or Baccalaureate).
• 1+ year of experience as a nurse in a clinical care setting.

Responsibilities

• Assess patients' physical, emotional, social, and environmental needs.
• Manage a caseload and develop, coordinate, and update individualized care plans.
• Complete initial assessments and provide ongoing evaluations during patient visits.
• Monitor patient progress and communicate changes with physicians, supervisors, and the care team.
• Educate patients, families, and caregivers about the plan of care and behavioral health support.
• Teach behavioral health care and symptom management to staff in partner care settings.
• Supervise and support Behavioral Health Aides and LPNs while ensuring quality care.
• Report safety concerns or other issues to supervisors as needed.
• Provide compassionate end-of-life support and complete bereavement assessments.

RN license
