Job summary

Elara Caring is seeking a Licensed Practical Nurse (Behavioral Health) to join our Hartford area team on a PRN basis. You’ll verify plans of care, deliver compassionate nursing visits, and collaborate with the RN Case Manager to support patients at home, including weekend runs.

Required is an accredited LPN program graduate with a valid state license and at least 1 year in clinical care; hospice or home health is preferred. Travel up to 50% and reliable transportation are expected.

Qualifications

• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• Current State License as a Licensed Vocational / Practical Nurse.
• 1 year of experience in a clinical care setting.
• Experience in a hospice or home health environment is preferred.

Responsibilities

• Verifies the Plan of Care prior to each visit and provides care according to delegation by RN.
• Incorporates patient care goals into nursing visits with proper documentation.
• Communicates findings to RN Case Manager and physician when appropriate.
• Implements patient safety plans and educates patients on self-care.
• Documents medication regimen and updates profile per procedures.
