Elara Caring is seeking a Licensed Practical Nurse for Behavioral Health visits in the Bridgeport area. The role supports patients in their homes, coordinating with RNs and care teams to provide effective, compassionate care.

Qualifications

• Current state license as an LPN.
• Graduate of an accredited nursing program (certificate, diploma, or associate).
• 1 year of clinical care experience.

Responsabilités

• Follow each patient’s individualized care plan and physician orders.
• Monitor patient progress and report changes to the care team.
• Assist patients and families with self-care techniques.
• Update care plans with RN Case Manager as needs change.
• Document medication regimens and update profiles per guidelines.
• Communicate with physicians, patients, families, and the care team for coordinated care.
• Help prevent avoidable hospitalizations through proactive care.
• Deliver compassionate, high-quality care for positive outcomes.
