Job summary

Catholic Charities Brooklyn and Queens seeks a Licensed Practical Nurse to join its team serving seniors and clients in Mental Health Clinics/PROS. The LPN provides wellness services, health monitoring, and care coordination to residents aging in place and clients in housing and clinics.

You will develop individualized care plans, perform assessments, lead health education, and collaborate with families and providers to support housing stability and treatment linkage.

Qualifications

• Must hold NYS LPN license.
• Certification in behavioral health or 3 years in BH population preferred.
• Excellent time management and communication skills.

Responsibilities

• Assess clients and establish care plans.
• Perform health monitoring and assessments.
• Lead health and wellness workshops.
• Coordinate care with families and providers.
• Document timely in paper and electronic records.
