Catholic Charities Brooklyn and Queens, Inc. seeks a Licensed Practical Nurse to support senior residents aging in place and clients in Mental Health Clinics/PROS.

The role includes medical and behavioral health care, health monitoring, and care coordination to maintain housing stability and connect clients to services. The LPN will develop individualized care plans, participate in crisis interventions, and collaborate with families and healthcare providers to address medical concerns and

Qualifications

• Must hold NYS Licensed Practical Nurse license.
• Certification in behavioral health or 3 years’ behavioral health experience.
• Proficient with computer systems and timely documentation.

Responsibilities

• Assess clients to establish a care plan.
• Perform health monitoring services and assessments.
• Lead workshops on health and wellness for clients.
• Advocate and educate clients.
• Coordinate care with families and other providers.
• Document all care in a timely manner (paper/electronic).
• Provide counseling and crisis intervention when needed.
• Refer clients to local community service providers.
