Job summary

Catholic Charities is seeking a Licensed Practical Nurse Health Services Coordinator to provide healthcare oversight and coordination for residents in our eight residential homes in St. Cloud area.

You will lead care coordination, oversee medication management, and collaborate with physicians, families, and staff to improve health outcomes and quality of life for adults with complex needs.

Qualifications

• Current Minnesota LPN license in good standing.
• Current CPR and First Aid certification.
• Minimum of four years of related experience.
• Experience supporting individuals with developmental disabilities or vulnerable populations preferred.
• Experience in residential services or healthcare coordination desirable.

Responsibilities

• Coordinate care across eight residential homes and coordinate healthcare services.
• Monitor health needs and track medical appointments.
• Oversee medication administration and ensure documentation.
• Collaborate with physicians, psychiatrists, pharmacists, families, guardians, and staff.
• Provide training and supervision to staff on healthcare procedures.
