Qualifications

• Bachelor's Degree in Social Service or related field required.
• Arizona Certification as LCSW/LPC/LISAC or PsyD preferred.
• Experience with behavioral health intake processes a plus.
• Excellent customer service and computer skills required.

Responsibilities

• Provide intake for Behavioral Health Program using required forms.
• Conduct telephonic and in-person intake assessments and referrals.
• Ensure timely response to client and referral needs.
• Perform clinical screening and appropriate risk assessments.
• Maintain documentation and reports for program and supervisors.
• Participate in team meetings and submit monthly reports.
