Job summary
Ascend Healthcare is seeking a Behavioral Health Intake Specialist to engage patients at partner Community Health Centers, guiding the intake process and documenting treatment plans. This role supports coordination across behavioral health, substance use, and medical services, and works with a multidisciplinary team to connect patients to appropriate care.

Qualifications
- Degree in a behavioral health field or 3 years’ experience in behavioral health OR completion of a Peer Support or BHT AHCCCS compliant training.
- Fingerprint Clearance Card or ability to obtain one.
- Proficient in electronic medical records, insurance verification and scheduling of services.
Responsibilities
- Lead new intakes and annual follow-ups within the CHC model.
- Coordinate care across behavioral health, substance use, and medical services with a multidisciplinary team.
- Support scheduled and walk-in patients and document treatment plans.
- Complete initial psychosocial intakes and updates to existing plans as needed.
