Job summary

ViaQuest is seeking a Behavioral Health Intake Specialist LPCC/LISW to join a multidisciplinary team. The role focuses on intake assessments and psychosocial evaluations, with opportunities to influence care at a system level.

Qualifications

• Licensed LPCC/LISW or related credential required.
• Master’s degree in social work, professional counseling, or marriage and family therapy.
• Medicare-eligible to provide intake and assessment services and 3+ years of behavioral health experience.

Responsibilities

• Provide clinical oversight for behavioral health intake activities.
• Conduct intake assessments, psychosocial assessments, and related documentation.
• Support SNF/LTC regional BHI team with resource coordination.
• Assist with Value-Based Care initiatives and interdisciplinary collaboration.
• Act as escalation point for Behavioral Health Coordinators.

Education

LPCC, LPCC-S, LISW, LISW-S or IMFT license

Master’s degree in social work or related field
