ViaQuest Mental Health is seeking a Hybrid Behavioral Health Intake Specialist (LPCC/LISW) to perform intake assessments and psychosocial evaluations. This full/part-time position offers a structured schedule, competitive salary ($65,000–$75,000) based on education and experience, and a monthly incentive program for productivity above expectations.

Responsibilities include clinical oversight, coordinating with an interdisciplinary team, and supporting SNF/LTC and Value-Based Care initiatives.

Formación

• Must be LPCC, LPCC-S, LISW, LISW-S or IMFT licensed.
• Master’s degree in social work, counseling, or marriage & family therapy.
• Medicare-eligible to provide intake and assessment services.
• Minimum of three years in behavioral health or related clinical experience.
• Experience in SNF/LTC, Value-Based Care, or Behavioral Health Integration preferred.
• Must be 18+ and have a valid driver’s license.

Responsabilidades

• Provide clinical oversight and intake/psychosocial assessments.
• Support regional BHI team in SNF/LTC settings.
• Assist in Value-Based Care initiatives and program development.
• Assume clinical leadership responsibilities as needed.
• Coordinate crisis response and maintain confidentiality.

Bilingual (plus)
