Job summary

ViaQuest is seeking a Behavioral Health Intake Specialist LPCC/LISW to join a multidisciplinary team. The role focuses on intake assessments and psychosocial evaluations, with opportunities to influence care at a system level.

This full/part-time position includes flexible scheduling and a compensation package aligned with education and experience. Ideal candidates are licensed in LPCC/LISW or IMFT, hold a master’s degree in a related field, and are Medicare-eligible with at least three years of

Responsibilities

• Provide clinical oversight for behavioral health intake activities.
• Conduct intake assessments, psychosocial assessments, and related documentation.
• Support SNF/LTC regional BHI team with resource coordination.
• Assist with Value-Based Care initiatives and interdisciplinary collaboration.
• Act as escalation point for Behavioral Health Coordinators.

Behavioral Health Intake Specialist LPCC/LISW

Full/Part Time Monday – Friday 9am-5pm or afternoons and evenings when needed

$65,000-$75,000 based on education and experience plus monthly incentive program for exceeding productivity expectations

Responsibilities may include

• Provide clinical oversight
• Intake assessments, conduct behavioral health intakes and psychosocial assessments
• Team resource support within a regional BHI team serving skilled nursing and long-term care (SNF/LTC) settings.
• Support Value-Based Care initiatives, and serve as a clinical
• Resource escalation point for Behavioral Health Coordinators
• Ideal for clinicians who enjoy systems-level impact,
• Interdisciplinary collaboration, and shaping a growing behavioral health program.

Requirements for this position include

• At least 18 years of age
• must be licensed as an LPCC, LPCC-S, LISW, LISW-S or IMFT
• Master’s degree in social work, professional counseling or marriage and family therapy;
• license must be maintained during employment.
• Clinical experience treating individuals with a variety of diagnoses and presenting issues is preferred.
• Must be Medicare-eligible to provide intake and assessment services and possess a minimum of three (3) years of behavioral health, case management, or related clinical experience.
• Experience working in skilled nursing facilities, long-term care settings, geriatric populations, Value-Based Care programs, and/or Behavioral Health Integration (BHI) programs is preferred.
• Experience conducting diagnostic assessments and behavioral health intake preferred. Prior team lead, supervisory, or clinical leadership experience is also preferred.
• Must be responsible, mature and flexible; must possess excellent decision-making, time management and communication skills
• Must be able to respond calmly to crisis situations; demonstrate the ability to direct others in a confident, respectful manner and exercise good judgment in interactions with residents, families, employees, peers and regulatory agency personnel.
• Must appropriately handle confidential information.
• Must have a valid driver’s license and reliable transportation.
• Bilingual language skills are a plus.
