Job summary

Prime Healthcare's St. Joseph Medical Center in Kansas City, MO is seeking an experienced LPN Intake Case Manager to assess and triage patients in behavioral health crises, coordinate admissions and placement, and support utilization review.

You will collaborate with the interdisciplinary team to ensure appropriate care, complete medical necessity documentation, coordinate insurance authorizations, and advocate for patients throughout intake and admission.

Qualifications

• Active LPN license in state.
• Minimum 2 years psychiatric nursing experience.
• BLS certification required upon hire.
• Management experience preferred.
• Experience in psychiatric emergency and triage preferred.
• Knowledge of Milliman Criteria and InterQual Criteria preferred.
• Knowledge of discharge planning regulations preferred.

Responsibilities

• Perform psychiatric assessments, crisis screenings, and triage; coordinate admissions and placements.
• Collaborate with the interdisciplinary team to ensure appropriate care.

Responsibilities

• Perform psychiatric assessments, crisis screenings, and behavioral health triage. Coordinate admissions, referrals, transfers, and patient placement.
• Collaborate with the interdisciplinary team to ensure appropriate care.
• Complete utilization review and medical necessity documentation.
• Coordinate insurance authorizations and support denial prevention.
• Serve as a patient advocate throughout the intake and admission process.

Qualifications

• Current and valid State LPN License
• At least 2 years of Psychiatric Nursing experience.
• BLS upon hire and maintained current.
• More than 2 years of management experience preferred.
• Experience in psychiatric emergency and triage preferred.
• Knowledge of Milliman Criteria and InterQual Criteria preferred.
• Knowledge of psychiatric medical necessity and management regulations in discharge planning.
