Job summary

KC CARE in Kansas City is seeking a Project Coordinator to manage day-to-day coordination of a federally awarded grant within the Behavioral Health team. You will oversee data collection, reporting, and timely follow-up on referrals related to the grant.

This role requires an associate degree in health sciences or related field, at least one year in social services or healthcare, and a commitment to serving a diverse population.

Qualifications

• One year of experience in a social services or healthcare related field.
• Demonstrated experience serving a diverse population.

Responsibilities

• Serves as the project coordinator for a federally awarded grant and provides the day-to-day operation support.
• Monitors grant awards and works with Director of Behavioral Health on ensuring spend down of grants.
• Completes all documents, forms, or reports required by the grant with support of the Grants Team.
• Coordinates monitoring of program outcomes and compiles data.
• Maintains accurate records and collection of data, which may include rosters, meeting notes, survey results, patient outcomes, etc.
• Prepares presentation materials for meetings, recruitment, and training on the federal program.
• Accepts client referrals for behavioral health services within the federal program.
• Documents client interactions thoroughly in a timely manner.
• Advocates, assesses, arranges, links, and/or integrates multiple services for clients.
• Participates or hosts community outreach events.
