Akido Labs, Inc. is seeking a Behavioral Health Care Manager to join the Enhanced Care Management team serving Inland Empire, CA.

You will support ECM-enrolled patients with complex behavioral health needs, collaborate with an interdisciplinary team, and ensure access to appropriate services and crisis resources. The role requires an MSW or MFT with California licensure preferred and residency in the region, with a focus on compassionate, community-based care across clinic and field settings.

Qualifications

• MSW or MFT required with CA licensure preferred.
• Local residence in Inland Empire region.
• Minimum 2 years of behavioral health case management experience.
• Bilingual English/Spanish preferred but not required.

Responsabilités

• Carry a panel of ECM-enrolled patients with behavioral health needs, including SMI and SUD, as part of a collaborative pod model.
• Conduct behavioral health screenings and assessments to inform individualized care plans with the team.
• Provide direct patient support using motivational interviewing, harm reduction, and trauma-informed approaches.
• Serve as the behavioral health resource for warm handoffs and crisis response protocols.
• Connect patients to mental health services, crisis lines, and community resources in the Inland Empire.
• Collaborate daily with the RN Care Manager, Care Coordinator, CHW, and Program Manager to coordinate whole-person care.
• Participate in daily huddles, weekly case conferences, and care planning meetings.
• Support transitions and warm handoffs to county behavioral health, inpatient, and crisis services.
• Communicate with PCPs and specialists regarding patient status and care coordination needs.
• Document encounters in Care Director in compliance with IEHP ECM program requirements and HIPAA.
• Assist audit preparation, quality improvement initiatives, and metric gap closure.
