Job summary

The Behavioral Health Care Management Specialist - ECM at Neighborhood-Healthcare will support enhanced care management patients with behavioral health conditions, SMIs, and substance use disorders using brief interventions and behavioral activation strategies. The role includes care coordination, appointment scheduling, and referral management across the ECM team.

This is a hybrid position with on-site expectations based on performance; typical shifts are 8:30 AM–5 PM or 9 AM–6 PM, Monday

Qualifications

• Excellent verbal and written communication skills required.
• Knowledge of medical terminology and health care terminology is preferred.
• Experience with community outreach and high‑risk populations is a plus.

Responsibilities

• Utilizes brief behavioral health interventions to improve patient health outcomes.
• Coordinates with healthcare teams and community resources for referrals.
• Educates patients and families about chronic conditions to improve health literacy.
• Documents care coordination activities and outcomes in the patient record.
• Maintains patient engagement using evidence‑based approaches like Motivational Interviewing.
• Participates in case reviews and collaborates with ECM team on patient care issues.
