WakeMed Health & Hospitals is seeking a Behavioral Health Discharge Specialist to coordinate discharge and transition planning, while providing essential clerical support to ensure timely patient disposition.

You will collaborate with BH clinicians, case managers, and external post-discharge providers, arrange transportation, and address social determinants of health to connect patients with outpatient services after discharge.

Competenze

• High School Diploma or Equivalent required.
• 1 year clinical discharge planning experience required.

Mansioni

• Coordinate discharge/transition planning services.
• Provide clerical support related to discharge execution.
• Coordinate with Case Manager, BH Licensed Clinician and others to obtain post-discharge services.
• Arrange transportation and address social determinants of health for outpatient follow-up.
• Proactively identify and mitigate potential delays in disposition.
