Job summary

Raleigh Recovery Center in Raleigh, NC is seeking a Case Manager to support the treatment team by coordinating care across providers and ensuring services align with the individualized service plan. This role emphasizes discharge planning and aftercare, with active collaboration among clinicians, families, and community resources.

The position requires a High School Diploma or equivalent, with preferred Bachelor's degree in Social Work or related field and prior behavioral health experience.

Qualifications

• High School Diploma or equivalent required.
• Bachelor’s degree in Social Work, Counseling or related human service field preferred.
• One or more years previous related experience in a behavioral setting preferred.
• Previous experience with the client population served by the facility preferred.
• Previous experience in case management preferred.

Responsibilities

• Develops and implements activities within their scope of practice.
• Collaborates with the client and client’s family or significant others to implement an effective service plan, explaining options to the team.
• Maintains the client’s comprehensive clinical record, including documentation of activities and progress toward recovery.
• Coordinates care with clinical team and providers to ensure ISP is implemented.
• Ensures discharge planning begins at admission and is coordinated with providers.
• Coordinates referrals for identified services and AHCCCS/SSI/SSD eligibility.
• Develops transition, discharge, and aftercare plans prior to discontinuation of services.
• Communicates client information to the treatment team and participates in team discussions.
