Job summary

Haywood Regional Medical Center in Clyde, NC is seeking a Discharge Planner to coordinate patient transitions from hospital to home or other care settings. You will work with patients, families, and the care team to ensure follow-up and resources are arranged.

The role emphasizes collaboration with nurses, social workers, and physicians, tracking forms, and building connections to housing, transportation, and community services.

Qualifications

• Bachelor's Degree preferred or 3 years of experience; or Master's degree
• Basic Life Support certification is required within 30 days of hire.
• Handle With Care within 30 days of Hire

Responsibilities

• Communicates effectively with patients, families, medical staff, nursing staff, and other caregivers when arranging follow-up care.
• Keeps track of all Medicare forms and consent forms.
• Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
• Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
• Interview and assesses patients and/or patient's family, caregivers, and/or legal representatives.
• Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
