JOB SUMMARY
- Provides, or assists in providing, psychosocial rehabilitation services to residents, including crisis management, counseling, assessment, treatment planning, discharge planning, case management, linkage, and referral.
QUALIFICATIONS
- Bachelor’s degree in Psychology, Social Work, or a related field.
- Minimum of one year experience working with the mentally ill or similar population.
- Proficient in reading, writing, and speaking English.
- Ability to work harmoniously with all personnel and deal tactfully with residents, families, visitors, and external agencies.
- Patience, tact, enthusiasm, and excellent communication skills.
- Ability to maintain professionalism in intense or crisis situations.
PHYSICAL REQUIREMENTS
- Ability to move intermittently and lift at least 25 pounds.
- Capability to participate in Crisis Prevention and Intervention (C.P.I.) techniques as trained.
- Must cope with mental and emotional stress and be free of physical limitations impacting job performance.
ESSENTIAL RESPONSIBILITIES
- Develop and implement clinically relevant interventions.
- Manage problematic behaviors and respond to crises following facility policies.
- Complete required documentation adhering to federal, state, and local regulations.
- Participate in team meetings, supervision, and continuing education.
- Report signs of resident abuse or neglect, and uphold resident rights.
- Work all shifts punctually and maintain high standards of practice.
- Assist in assessments, treatment planning, and documentation such as M.D.S. and R.A.P.S.
- Provide individual and group counseling, case management, and discharge planning.
- Assist in managing the milieu and mealtimes, and participate in clinical supervision.
- Support activities and collaborate with other departments to manage resident behaviors.
- Perform other duties as assigned.
