Job summary

Union Community Care is seeking a Behavioral Health Counselor to provide therapy and brief interventions within a multidisciplinary team. You will conduct diagnostic evaluations and treatment planning for individuals and families in a community health setting.

The role requires a current PA clinical license and strong communication skills. You will participate in team meetings, maintain documentation per standards, and contribute to maintaining comprehensive care for patients seeking addiction

Qualifications

• Current PA license to practice psychology or social work/therapy.
• Strong communication skills, both written and verbal.
• Ability to work within a multidisciplinary care team.

Responsibilities

• Provide behavioral health counseling to individuals and families.
• Work with Team-Based care unit to maximize level of care.
• Perform intake histories for addiction treatment and plan visits.
• Collaborate with clinicians and refer to external partners as needed.
• Document evaluations, progress notes, and discharge procedures.
• Participate in staff meetings and maintain clinical competencies.
