Job summary

ChesPenn seeks a Behavioral Health Consultant Associate to join the medical team, providing integrated behavioral health care during and after medical visits under the supervision of the Behavioral Health Manager.

Requirements include a Master’s in Social Work, LSW licensure or eligibility in Pennsylvania, and Spanish-language ability is a plus. Responsibilities include triage, consultation, and documentation in the electronic medical record.

Qualifications

• Completed Master’s Degree in Social Work.
• Licensed Social Worker (LSW), or eligible in Pennsylvania. Required.
• Clinical track to LSCW and internship documentation required.
• This position regularly serves Spanish-speaking patients and the ability to communicate professionally in Spanish is a plus.

Responsibilities

• Provide behavioral health consultation services to individuals, families, and groups as needed based on patient and medical practice needs.
• Work as a member of the healthcare team providing behavioral health services to pediatric and adult patients with behavioral health issues, substance use disorder and/or chronic medical conditions.
• Crisis assessment, intervention, and safety planning as needed.
• Perform targeted strengths-based assessment and evaluation, including diagnostic impressions and functional status focused on the presenting problem.
• Provide timely and succinct feedback to Providers regarding consultation findings and recommendations.
• Concise documentation in the patient’s electronic medical record.
• Triage and refer patients to specialty mental health care when appropriate.
• Provide brief follow-up, including relapse-prevention education.
