Job summary

Responsibilities

• Accountable for provision of social work/therapy services within Primary Care Offices and virtually or telephonically.
• Perform psychosocial assessments, crisis intervention, individual therapy, admission coordination, utilization review, and liaison services.
• Work independently and as part of the interdisciplinary treatment team.
• Conduct ongoing assessment, evaluation and treatment planning from admission to discharge.
• Demonstrate knowledge of productivity standards and maintain them.
• Develop professional relationships with community agencies.
• Provide one-to-one therapy and facilitate group therapy.
