Job summary

Carle Health is seeking a Qualified Mental Health Professional (QMHP) to provide behavioral health assessment, intervention, and consultation in outpatient settings in Illinois. Under supervision, you will plan treatment modalities, coordinate treatment plans, and provide crisis evaluation when needed.

The role requires a Master's degree in a related field and supervised clinical experience (1000 practicum hours or 1 year under QMHP supervision).

Qualifications

• Master's degree in Counseling, Social Work, Psychology, or related field.
• 1,000 practicum hours or internship under supervision, or 1 year of clinical experience under QMHP supervision.
• Ability to apply critical thinking and problem-solving to diverse client situations.
• Ability to assess, diagnose, and develop treatment plans.
• Strong written and oral communication skills.
• Ability to plan and organize work and coordinate care.

Responsibilities

• Performs diagnostic assessments and determines immediate treatment plan needs with patients; requests medical consultation when indicated.
• Completes treatment plans with patients, identifying goals aligned with assessment findings.
• Provides behavioral health therapy to individuals, families, and/or groups.
• Coordinates with Emergency Response Service and crisis teams for high-risk clients.
• Maintains records in accordance with organizational policies and regulatory bodies.
• Acts as liaison between community, primary care, courts, and managed-care entities.
