Job summary

Carle Health is seeking a qualified mental health professional to provide outpatient behavioral health assessments, interventions, and crisis evaluation under supervision. Qualifications

• Master's degree in social work, psychology, counseling, or related field required.
• Completion of 1,000 practicum hours and/or internship under clinical supervision or one year of supervised QMHP experience.

Responsibilities

• Performs diagnostic assessments and determines immediate treatment plan needs with patients; requests medical consultation when indicated.
• Completes treatment plans with patients, identifying goals/objectives linked to assessment.
• Provides behavioral health therapy to individuals, families and/or groups in need.
• Coordinates with Emergency Response Service and crisis teams when clients may be a danger to self or others.
• Maintains records per UnityPlace policies and regulatory/accreditation agencies.
• Acts as liaison between community and patient needs; coordinates treatment goals with community agencies and care providers.
• Meets clinical productivity standards.
• Meets compliance standards for all documentation and charts.
• Participates in in-service and competency training.
• Prepares special reports on activities and group work.
