Job summary

Lifelong Access is seeking a Qualified Mental Health Professional (QMHP) in Illinois to provide assessment, treatment planning, therapy, and related mental health services to clients in our behavioral health program. Qualifications

• Master’s or doctoral degree in counseling and guidance, rehabilitation counseling, social work, vocational counseling, psychology, pastoral counseling, or a related field with supervised direct service hours.
• Strong understanding of confidentiality and ethical guidelines.
• Must be at least 21 years of age and able to pass background checks.

Responsibilities

• Perform clinical assessments and develop client-centered treatment plans.
• Provide individual and group therapy to supported individuals.
• Ensure billable services are documented and submitted per regulations.
• Provide training to agency staff to support treatment plan implementation.
• Implement, review, and modify treatment plans as needed.
• Collaborate with the treatment team and participate in agency-wide activities.
• Complete case notes and maintain files per regulations and policies.
