Qualifications

• Master's Degree in Social Work, Psychology, Counseling, or a Related Field.
• Completed 1,000 hours of practicum and/or internship under clinical supervision, or one year of clinical experience under supervision as QMHP.
• Demonstrates critical thinking, problem-solving, ability to assess and diagnose, develop treatment plans, strong written and oral communication, and understanding of ethical standards in behavioral healthcare.

Responsibilities

• Performs diagnostic assessments, collects diagnostic information, and determines immediate treatment plan needs with patients. Requests medical consultation as needed.
• Completes treatment plans in conjunction with patients, identifying goals/objectives linked to assessment.
• Provides behavioral health therapy to individuals, families and/or groups with behavioral health needs.
• Coordinates with Emergency Response Service and crisis teams for at-risk clients.
• Maintains records in accordance with Carle Health policies and applicable regulations.
• Acts as a liaison between the community and patient needs, coordinating goals with community agencies and care providers.
