Job summary

Community Bridges, Inc. is seeking a Behavioral Health Clinician to join its crisis, inpatient, and transition programs.

The clinician will coordinate with patient care technicians, registered nurses, and behavioral health medical providers to maintain a safe environment and deliver quality care to individuals presenting for services. The clinician will complete behavioral health assessments and service plans, document services provided, and coordinate discharges.

Qualifications

• Bachelor's degree in a field related behavioral health required; in lieu of a bachelor’s degree, a high school diploma or GED and at least 1 year of full-time health care related work exposure considered.

Responsibilities

• Complete behavioral health assessment and service plan for all individuals admitted.
• Document provided services and coordinate discharges from CBI programs.
• Coordinate care with family or other significant others.
• Collaborate with Patient Care Technicians, Registered Nurses and Behavioral Health Medical Providers to ensure safe, high-quality care.

Behavioral Health Clinician

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.

Job Summary

The Behavioral Health Clinician (BHC) is a member of the interdisciplinary treatment team in CBI Crisis, Inpatient and Transition Point programs. The Behavioral Health Clinician coordinates with the Patient Care Technicians (PCT), Registered Nurses (RN), and Behavioral Health Medical Providers (BHMP) to ensure a safe environment and that quality care is provided to all presenting for services. The Behavioral Health Clinician completes the behavioral health assessment and service plan for all individuals admitted to their assigned program. The BHC documents provided services and coordinates discharges from CBI programs. The BHC coordinates care with the family or other significant others.

Skills/Requirements:

• Bachelor’s degree in a field related behavioral health required. In lieu of a bachelor’s degree, a high school diploma or GED and a minimum of one (1) year of full-time health care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations will be considered.
