Job summary

Community Bridges, Inc. (CBI) is seeking a Behavioral Health Clinician to join our interdisciplinary team across Crisis, Inpatient, and Transition Point programs in Arizona.

The clinician coordinates with PCTs, RNs, and BHMPs to ensure a safe environment and delivers behavioral health assessments and service plans for admitted individuals. The role involves documenting services, coordinating discharges, and communicating with families to ensure continuity of care, with preference for candidates

Qualifications

• Bachelor’s degree in a field related to Behavioral Health. Master’s degree preferred but not required.
• 1–3 years of full-time healthcare-related work experience in behavioral and medical crisis exposure is preferred but not required.

Responsibilities

• Coordinate with PCTs, RNs and BHMPs to ensure a safe environment and quality care.
• Complete behavioral health assessment and service plan for admitted individuals.
• Document provided services and coordinate discharges from CBI programs.
• Coordinate care with family or other significant others.

Behavioral Health Clinician

CBI is the premiere non‑profit fully integrated healthcare provider of substance use and behavioral health programs in Arizona, including prevention, education and treatment using cutting‑edge, nationally recognized, evidence‑based models.

Job Summary

The Behavioral Health Clinician (BHC) is a member of the interdisciplinary treatment team in CBI Crisis, Inpatient and Transition Point programs. The Behavioral Health Clinician coordinates with the Patient Care Technicians (PCT), Registered Nurses (RN), and Behavioral Health Medical Providers (BHMP) to ensure a safe environment and that quality care is provided to all presenting for services. The Behavioral Health Clinician completes the behavioral health assessment and service plan for all individuals admitted to their assigned program. The BHC documents provided services and coordinates discharges from CBI programs. The BHC coordinates care with the family or other significant others.

Skills/Requirements

• Bachelor’s Degree in a field related to Behavioral Health. Master’s Degree is preferred but not required. Preference given to Master’s Degree in field related to behavioral health and/or has been licensed at the associate level with The Arizona Board of Behavioral Health Examiners as Psychologist, Social worker, Counselor, Marriage and family therapist or Substance Abuse Counselor.
• 1-3 years of full-time Health Care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations is preferred but not required.
