Community Bridges, Inc. in Arizona is seeking a Behavioral Health Clinician to join our crisis, inpatient and transition programs.

The BHC coordinates with Patient Care Technicians, Registered Nurses and Behavioral Health Medical Providers to ensure a safe environment and quality care for all clients. The BHC conducts behavioral health assessments, develops service plans, documents services and coordinates discharges, while engaging with families.

Qualifications

  • Bachelor’s degree in a field related to Behavioral Health is required.
  • Master’s degree is preferred but not required.
  • Licensure at the associate or independent level is preferred.

Responsibilities

  • Complete behavioral health assessments and service plans for admitted individuals.
  • Coordinate care with PCTs, RNs, and BHMP to ensure a safe environment and quality care.
  • Document provided services and coordinate discharges from programs.

Job Summary

The Behavioral Health Clinician (BHC) is a member of the interdisciplinary treatment team in CBI Crisis, Inpatient and Transition Point programs. The Behavioral Health Clinician coordinates with the Patient Care Technicians (PCT), Registered Nurses (RN), and Behavioral Health Medical Providers (BHMP) to ensure a safe environment and that quality care is provided to all presenting for services. The Behavioral Health Clinician completes the behavioral health assessment and service plan for all individuals admitted to their assigned program. The BHC documents provided services and coordinates discharges from CBI programs. The BHC coordinates care with the family or other significant others.

Skills/Requirements
  • Bachelor’s Degree in a field related to Behavioral Health. Master’s Degree is preferred but not required. Master’s Degree in field related to behavioral health and/or has been licensed at the associate level with The Arizona Board of Behavioral Health Examiners as Psychologist, Social worker, Counselor, Marriage and family therapist or Addiction Counselor preferred.
  • 1-3 years of full-time Health Care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations preferred.
  • Valid identification required.
  • Licensure at the associate or independent level with The Arizona Board of Behavioral Health Examiners as Psychologist, Social Worker, Counselor, Marriage and family therapist or Addiction Counselor is preferred.
