Job description

This is a field-based position that requires home visits in the Florence, SC region. REQUIRES candidates to be residents of South Carolina.

Responsibilities:

The Field-Based Remote Behavioral Health Case Worker manages and coordinates care, services, and social determinants of health for Members with behavioral health conditions and complex medical needs. Serving as the primary contact for the care team, which includes Members, physicians, and community supports, the worker guides Members toward optimal health outcomes. Utilizing assessment, communication, critical thinking, and clinical skills, the worker identifies issues, gaps, and barriers to care. Responsibilities include:

1. Supporting Members during care transitions through assessment, care coordination, education, referrals, and evaluating the effectiveness of discharge plans.
2. Reviewing medication lists and coordinating pharmacy needs with community pharmacists to ensure medication reconciliation.
3. Evaluating, monitoring, and updating care plans during follow-ups based on Member progress and preferences.
4. Establishing contacts to collaborate with community, medical, and behavioral health teams.
5. Maintaining accurate documentation of Member interactions in electronic care management platforms.
6. Monitoring utilization, coordinating services, making referrals, and escalating quality issues.
7. Developing knowledge of care management platforms, policies, procedures, and community resources.
8. Presenting cases at care management rounds and following up with providers.
9. Fostering a positive, collaborative, and culturally sensitive workplace environment.
10. Conducting face-to-face visits at residences, provider offices, hospitals, or community locations for education or assessment.
11. Performing other duties as assigned.

Education/Experience:

• Bachelor's Degree in Human Services or related field required; Master's preferred.
• At least 2 years of experience working with SMI members aged 6 and above with conditions like Psychotic Disorders, Bipolar Disorder, or Major Depression.
• At least 2 years of social services experience engaging with clients, assessing needs, connecting to services, and monitoring progress.
• Proficiency in MS Office, internet applications, and electronic medical records (e.g., Jiva).
• Ability to manage a caseload of up to 60 members and support transitions of care.
• Strong documentation skills, accuracy, and adherence to methods.
• Experience in developing care plans, assessing situations, and teaching self-management, with effective communication skills for individuals and groups.
