Job description

This role is field-based remote and will require travel within the Orangeburg area to service our members. Candidates must live in or very near Orangeburg, SC to qualify.

Responsibilities:

The Field-Based Remote Behavioral Health Case Worker manages and coordinates care for Members with behavioral health conditions and complex needs. Serves as the primary contact for the care team, including Members, physicians, and community supports, to promote health and self-management. Responsibilities include:

  • Supporting Members during care transitions through assessment, coordination, education, referrals, and evaluating care plans.
  • Reviewing medication lists and coordinating pharmacy needs with community pharmacists.
  • Monitoring and updating care plans based on Member progress and needs.
  • Establishing contacts with community, medical, and behavioral health teams for collaboration.
  • Documenting Member interactions accurately in electronic care management platforms.
  • Monitoring utilization, coordinating services, making referrals, and escalating quality issues.
  • Gaining knowledge of care management platforms, policies, procedures, Member benefits, and regulatory requirements.
  • Participating in care management rounds and following up with providers.
  • Fostering a positive, collaborative, and culturally competent work environment.
  • Conducting face-to-face visits at Members’ homes, providers’ offices, hospitals, or community locations.
  • Performing other duties as assigned.
Education/Experience:
  • Must be a resident of or near Orangeburg County, SC.
  • Bachelor’s degree in Human Services or related field required; Master’s preferred.
  • At least 2 years of experience working with individuals with Serious Mental Illness (SMI), including conditions like Psychotic Disorders, Bipolar Disorder, or Major Depression.
  • 2 years of social services experience engaging with clients, assessing needs, and monitoring progress.
  • Proficiency in MS Office, internet applications, and electronic medical records (e.g., Jiva).
  • Ability to manage a caseload of up to 60 members and support care transitions.
  • Strong documentation skills and experience in care planning and health education.

Must be willing to travel within the region, access members’ homes and community locations, and possess reliable transportation, a valid driver’s license, and auto insurance.
