The Behavioral Health Caseworker is responsible for coordinating services and addressing social determinants of health for Members with behavioral health conditions. Under the direction of the Population Health Manager and Supervisor, the Behavioral Health Caseworker ensures the provision of services in the least restrictive setting and transitions support across and between care settings; connects members to services that promote community living and helps to overcome any barriers to health; establishes points of contact to collaborate with community, medical, and behavioral health teams.

The Behavioral Health Caseworker meets with assigned Members in their homes or community settings. They will develop an understanding of each Member’s circumstances and needs, assess available services, and create a person-centered plan to address behavioral health needs. For physical health needs, the caseworker will consult with a registered nurse care manager.

The Behavioral Health Case Worker will:

  1. Support Members during behavioral health transitions through person-centered assessment and coordination of resources. Collaborate with Members during transitions, provide referrals, and link to community resources while continually evaluating the person-centered plan.
  2. Review medication lists with members and coordinate pharmacy needs to support medication adherence.
  3. Monitor and update the person-centered plan through regular follow-ups, preferably in the Member’s home. The plan should be evaluated based on Member/caregiver progress, needs, and preferences.
  4. Establish points of contact to collaborate with community, medical, and behavioral health teams.
  5. Maintain accurate documentation of Member interactions in ACFC electronic care management platforms.
  6. Develop knowledge of ACFC platforms, care management programs, policies, procedures, workflows, Member benefits, community resources, and regulatory requirements.
  7. Support a positive, inclusive workplace environment, collaborating and sharing knowledge to serve our diverse Member population.
  8. Conduct face-to-face visits at Member residences, provider offices, hospitals, or community locations for education and assessment.
  9. Perform other duties as assigned.

Education/Experience:

  1. Bachelor’s Degree in social work, psychology, sociology, early childhood education, child development, criminal justice, or a related field, or in another field.
  2. Required Bachelor’s Degree in Human Services with 45 hours of Behavioral Health training OR Master’s Degree in social work, nursing, rehabilitation, psychology, or a related health and human services area with documented experience with the Member population.
  3. Refer to the State Requirement Document and Market Contract for additional requirements.
  4. At least 2 years of professional experience working directly with individuals affected by severe mental illness (SMI), including conditions such as Psychotic Disorders, Bipolar Disorders, or Major Depression.
  5. At least 2 years of social services experience, engaging with clients, assessing needs, connecting to services, and monitoring progress.
  6. Proficiency in MS Office, internet applications, electronic medical records, and care management platforms (e.g., Jiva).
  7. Strong documentation skills emphasizing accuracy and adherence to methods.
  8. Experience assessing members’ situations, developing action plans, and teaching self-management. Skilled in working one-on-one and communicating health information effectively.
  9. Willingness to travel within assigned regions, access members’ homes and community locations, and possess reliable transportation, a valid driver’s license, and auto insurance.
